Missed Appointments: You will be charged a Missed Appointment fee of $25.00 for any missed appointments or late cancellations (less than 24 hours notice). It is under the discretion of the owner to waive this fee based on the circumstances
Payment: The policy of New Life is to collect all payment at the time services are rendered. For your convenience we accept cash, VISA, Mastercard, and most major credit cards. We do not currently accept major insurance plans.
Monthly Memberships: Our month-to-month memberships not require a minimum month contract. Our policy notes that all cancellations of the monthly memberships must be made 7 days or more in advance of the auto draft date and you will receive a 100% refund. Cancellations made within 3 - 6 days will incur a 20% fee of total monthly charge. Cancellations made within 48 hours to the event will incur a 30% fee of the total monthly charge
Refunds: Prepayment of programs are nonrefundable. Prepayment is only offered to patients in an effort to provide a discounted cost of services. Payment to our office for monthly or prepaid plans includes only the necessary office visits and medications required for your particular goals and treatment plan. All dispensary products including wellness products must be paid at the time of service. There are no refunds
on products once they have left the pharmacy
Dismissal Policy: There are a few reasons that a patient may be dismissed from our practice: Failure to keep scheduled appointment or failure to meet financial obligations.
